The requirements are:
• The child must be 5 years old on or before Aug. 31, 2011;
• Copy of a child’s birth certificate;
• School entrance health certificate;
• Copy of health insurance or Medicaid;
• Map to residence;
• Copy of legal guardianship, if applicable
