A kindergarten student applicant must be 5 years old on or before Aug. 31, 2011. The following documents are required: birth certificate, school entrance health certificate (updated), current school accident/ health insurance/ medical (updated), map of residence, and legal adoption/ guardian document) if applicable (CNMI/U.S. court issued).
